Mediante la suscripción de la presente ficha y de acuerdo con lo dispuesto en la Ley de Protección de Datos Personales Nº 29733 y su reglamento, declaro mi consentimiento expreso, previo, libre e informado para que mis datos facilitados en ella sean mantenidos en una Base de Datos, debidamente inscrita por la Derrama Magisterial ante la Autoridad Nacional de Protección de Datos Personales. Declaro conocer mis derechos de acceso, rectificación, cancelación, supresión y oposición como titular de dichos datos, autorizando el tratamiento por parte de Derrama Magisterial para efectos estadísticos, la promoción y difusión de los servicios que brinda así como la transferencia de la información legalmente permitida a terceros, dentro y/o fuera del territorio del Perú.

According to the provisions of the Law for the Protection of Personal Data and its regulations, I hereby expressely grant my previous, free and informed approval for the personal data provided in this form to be kept by Derrama Magisterial in a data base to be duly registered before the Peruvian Authority for Personal Data Protection. I also declare to be fully aware of my rights to access, modification, cancellation, deletion and opposition on such personal data, and hereby grant my authorization to Derrama Magisterial to use and manage it for purposes of statistics, marketing and advertising of their services, and communication of legally permitted information to third parties, within and/or abroad Peruvian Territory.

1. El servicio de alojamiento deberá ser pagado en su totalidad en el check in.  / The accommodation service must be paid in full at check in.
2. Todo huésped tiene que presentar su documento de identidad y ser registrado en el check In. / All guests must present their identity card and be registered at check in.
3. Para tener la cuenta abierta en la habitación se le solicitará de manera anticipada una garantía (en efectivo o con tarjeta de crédito), la cual le permitirá realizar cargos para ser pagados al final de su estadía, caso contrario todo consumo tendrá que ser pagado al contado en efectivo o con tarjeta de crédito o débito. / To have the account opened in the room you will be asked in advance for a guarantee (cash or credit card), which will allow you to make  charges  to  be paid at the end of your stay, otherwise all consumption will have to be paid in cash or by credit or debit card.
4.  No está permitido fumar dentro de las habitaciones, de  acuerdo  al  Decreto  Supremo N° 28705, de no  respetar  esta  política, tendrá  que pagar  una penalidad de 200 USD. / Smoking is not allowed inside the rooms, according to the Supreme Decree N° 28705, if you do not respect this policy, you will have to pay  a  penalty of 200 USD.
5. El horario de visitas del establecimiento es entre las 08:00 am a 07:00 pm, con un tiempo máximo de 30 minutos. De  lo  contrario,  se  procederá  a realizar el cobro de 30 USD por persona adicional. / The establishment’s visiting hours are between 08:00 am to 07:00 pm, with a maximum time of 30 minutes.  Otherwise, the  payment  of  USD  30  per additional person will be made.
6. Si trae consigo dinero en efectivo u objetos de valor, cuenta con una caja de seguridad en la habitación, la empresa no se responsabiliza por la pérdida o deterioro del contenido de la misma, para más información consulte con el área de recepción. / If you bring cash or valuables, has a safe in the room, the company is not responsible for the loss or damage of the contents of it, for more information check with the reception area.
7.  El establecimiento no se responsabiliza por accidentes ocurridos por negligencias de huéspedes, actos fortuitos o por no seguir  las  políticas internas de la empresa. / The establishment is not responsible for accidents occurring due to negligence of guests, acts  of  God  or  failure  to  follow  the  internal  policies  of  the company.
8. El huésped se hace responsable por cualquier deterioro causado dentro de las habitaciones. / The guest is responsible for any damage caused inside the rooms.
9. Si el huésped se retira antes de la fecha establecida, no habrá reembolso por las noches no pernoctadas. / If the guest leaves before the established date, there will be no refund for nights not spent.
10. Las áreas de relax serán usadas en los horarios establecidos. / Relax areas will be used at established times.
11. El cliente pagará una penalidad de 15 USD si llegara a perder la llave de la habitación. / The client will pay a penalty of 15 USD if the room key is lost.
12. El huésped declara que no está ingresando al hotel con armas de fuego, materiales explosivos, inflamables, estupefacientes, o sustancias similares. / The guest declares that he/she is not entering the hotel with firearms, explosive materials, flammable materials, narcotics, or similar substances.
